Interpreting FAR's can be very frustrating, but let's take a deep breath and read it slower. Although we often simplify regs for operational purposes by paraphrasing such as "I only need an alt if its bad wx". 135.223b actually says you are always required to have an alt UNLESS the forecast conditions will be AT LEAST 2000 and 3(or 1500 and 2 added to lowest circling or other applicable approach criteria whichever is GREATER). The key words that indirectly address Temporarily or occasionally are the words "AT LEAST". If you have a forecast of 2 miles with occasional 1/4 mile vis then you don't have "AT LEAST" 2 miles vis, you have "AT MOST" 2 miles vis and "AT LEAST" 1/4 mile vis. I hope that helps and ends your frustration.
