I always thought the NDB into Chattam, MA is pretty tough. There is not much to hit, but look at the full procedure and the missed. It's nick named "the whipping post". Its not as tough as some of those already listed though.

In the USA, Aspen, Colorado (and Eagle) were beat into at Bombardier. I've been both places many times and have seen that their approaches are full of cumulus granite. Screw up there, you die. Some where in your report it will say "CFIT". If an approach is "day time only" that's a big hint that it's a documented killer. On take off from either place, if you lose an engine or have a TR deploy at V1 and you better have all your stuff together, not just some.
