Can't wear the mask if people are smoking. On 135 flights, smokers can't fly above FL350. FL410 for 91k.

Smoking affects fuel consumption and range. On a positive side, you get exposed to less UV.

No smoking while on O2? I'm not familiar with this one but don't have the FOM either.
 
No smoking while oxygen is in use. It's in most of the manuals I no of.
Of course they wouldn't repeat it in the FOM.
No ACP has ever reprimanded me for saying we can't go at FL430 because the brief says Mr. Big likes to smoke.
Does any one really fly that many smokers?
 
I've never seen a prohibition of smoking in the back of the airplane while the crew is using supplemental O2 under Part 91, 91K, or 135. If you can find the regulatory reference, I'd be ecstatic to see it.

In the fractional world, it's "their" airplane. If they want to smoke back there, they can. Whether we're at 35,000 or 41,000, or 51,000. Sad, but true. Fortunately, it is rarely an issue any more since I get maybe one or two smokers a year.
