The NTSB factual report just arrived on the
fatal crash of the Gulfstream G-1159A (GIII), N85VT, which impacted a light
pole near Houston Hobby Airport, on November 22, 2004.

The NTSB cautioned, "The reader of this report is cautioned that the
transcription of a CVR tape is not a precise science but is the best product
possible from an NTSB group investigative effort.The transcript, or parts
thereof, if taken out of context, could be misleading. The attached CVR
transcript should be viewed as an accident investigation tool to be used
inconjunction with other evidence gathered during the investigation.
Conclusions or interpretations should not be made using the transcript as
the sole source of information."

Why was the FO continually reassuring the CA, even when the CA and pilot flying felt things were screwed up? That dude had diarreha of the mouth.
 
Not to try afix any blame, just found it chilling to read the part of the transcript in which the non-flying pilot apparently programs the FMS :confused::

In relation to this approach, the VOR is offset to the left of the runway at the back end of the airfield. If the crew was flying with the NAVs tuned (and a centered CDI) tuned to the VOR, that would explain the aircrafts offset on the approach, but not the descent below mins although both mentioned a lack of vertical course guidance not long before the transcript ends. :confused:

I must admit, it's sometimes hard to make the go around decision. I've probably landed out of an approach I shouldn't have continued. But this just reinforces the idea that if your gut is saying "Go Around", then go around and sort it out in the air.
 
Thanks for the post 2000flyer.

This accident makes me realize how blessed I have been in my career. I was able to fly one Lear for most of my Lear time, and even though my current employers MD80 fleet is not exactly standard, they are all close enough to
have allowed me to avoid this kind of deal.

It would seem that a lack of familiarity with the aircraft killed these guys.

I wonder if this will result in a new rule requiring 135 operators to maintain fleet standardization?

BTW, BigDuke6. I think that they were just about to start a go around. Unfortunately, they didn't figure out the improper programming until a little too late.
 
After re-reading the transcript a couple of times, one of the gnawing feelings I have is that the pilot flying didn't seem familiar with the aircraft. Instead of recognizing the error, he stammered around a bit leaving me with the feeling his eyes were searching the panel looking for a frequency or mode control. The NFP, however, was confident throughout the hole procedure up until the end..."you're squared away now." It just makes me wonder how current or proficient the FP was and how easy it was to talk him into continuing a seriously flawed approach. The guy must have some good experience as it sounded as if his "day job" was flying a BAC.

I don't know about the rest of you but in my book (and practice), any...ANY...time you're inside the FAF and something out of the usual distracts you enough to start asking questions, GO MISSED! The FP in this accident had internal alarms going off time and time again and yet he continued. Why? And, the NFP...the instant he changed the frequencies from VOR to LOC, especially without telling the FP BEFORE he did so, is grounds for immediate missed approach.

I won't try and second guess the quality of either pilot. However, from the get-go, this was a "we're gonna land" approach considering immediately after receiving the ATIS they began talking about tower "giving them a break," and telling tales of days gone by when they made it in at other times in their lives.

Enigma is spot on, in his/her observations. Flight safety's long standing motto about the best safety device in any A/C being a well trained pilot just doesn't cover all the bases. Time in type is an even better measure of safety, but time in specific aircraft with identical equipment installation, or a singular tail number, trumps all.

This accident really hits home. Unfortunately, we'll never see any regulatory changes to prevent this scenario from occuring again and again.
Why ? Because to achieve this familiarity, operators would have to hire more pilots and spend more money. In the money vs. safety equation, money always wins.

I am still puzzled why they continued with the vis at 1/4.
I may have missed it in the transcript.???
