Show us the research that these costs materialize during your working years. They don't. The only action that saved you money is not using healthcare.

Where did I say I didn't use healthcare? I get a comprehensive physical every 2 years (and not just a pilot med, but a real one). I contribute $4K/year pre-tax to my HSA to fund future healthcare expenses. Your reading comprehension makes me think you need a cat scan.

All Three of your sources use BMI as the factor for Obesity, which has been proven to be an inaccurate measure of Health. e.g. a 5'10" 210lb body builder is Obese but has 8% body fat, a 5'10 180lb office worker with 25% body fat is not.... Find some better sources.

Getting a physical each year is NOT accessing the health care system. It is a wellness check that is purely discreationary. Recommended but discreationary.

Your research (which I imagine you just located) does not differentiate between Genetics and Behavior. According to your study a FAT GUY is A FAT GUY and I was fat in high school because I was 6' 2" and weighed 220lbs. Forget that I ran 40 miles a week, played football and baseball and ran track. Guess where most of medical cost have originated from?

My hip surgery was because I was fat? Nope it was because I pinched a nerve running in the hills of Newport Coast for Baseball and football instead from not eating granola yesterday.

You are just lucky not to have had a minor or major accident, injury or illness in the past 7 years so that you could accumulate your $28. That luck is not scaleable or a plan.
