I'm not an active duty type, but i think it would be sufficient to include it as one entry and list your most recent unit.
I don't think it would hurt you to list them, as long as you didn't bust a LOT of checkrides. I think the majority of pilots busted something at UPT at sometime... FWIW...
Bummer. Sounds like the best way to go about it is to write a letter, explaining the full time ART status honestly, and see if they will grant you leave anyway...
So when they ask for first class medical, are they talking issued within the last 12 months? My medical is originally issued first class, but I'm at the 18 month mark due to mil leave. Military aviation medical is current of course, not that it matters...
I know at least one that did at a regional. Not sure how, since there are no "orders" and it was a technician job.....

If you are simply wanting to become a pilot in the guard, then yes, civillians can apply, you do not have to be prior service. If hired, you will become a member of the military and have to go through the normal military...
