Cleared for takeoff and I understood to turn left to 190. Tower had apparently instructed to continue straight ahead. I missed the call, but the FO, acknowleged it. FO got busy and didn't notice the turn. There was a loss of separation with someone, whom we identified and had a visual on, and transmitted the visual with with ATC.

Anyway, FSDO is involved at some point. NASA forms are filed. Any idea how this might play out?

You will probably get a "letter of correction" that stays on your record for 2 years. Does not show up on PRIA, however. Only the FAA can access it. I had the same thing happen.

However, This is reportable to a new/future employeer. If you withold the info and they find out about it from FAA records..You are screwed!

Further research found this

"Professional pilots can take some measure of relief from the fact that under the Pilot Records Improvement Act (PRIA), Warning Notices and Letters of Correction aren't released to an airline. However, the information will be released to an airline, or anyone else, that makes a Freedom of Information Act (FOIA) request."
