Where Can I Get the "NASA" form?

I was flying into Lakeland yesterday, and there was traffic to my left heading for base, as was I. So I made somewhat of a wide pattern, and tower said something about it..then someone chimed in "embry riddle trained". I ended up landing close behind the guy anyway....so I don't see the problem with me spacing things out....

GRrrrrrrrrrrrrrr...I have no riddle flight training....

I read the quote on a post from Mini in a different forum and had to look up the original message, in pure disbelief that someone actually said that.

I apologize....I know that's X minutes you'll never get back, but I couldn't resist...funniest post I ever read.
