IMHO, your actual should equal roughly 10% of your ttl time. Show N numbers and type. Date, and year. Just make sure it makes sense.

I know we here at DAL like to see approaches and actual, night and landings in day and night. By day is fine, by month is not something I would do. Some have gotten away with it, but I know a few interviewers that hate that, as they think it shows laziness.
Also, technique, but all times in the bottom are in pencil so that you show a willingness to change something if they do not see that as time. I know UAUA like this back in the 90's.
