They ask "have you EVER received a moving violation" or something to that effect. Am I supposed to remember a speeding ticket from 12 years ago? I hate to click yes when it may not even show on my record. Any ideas?

I have the same problem. I have my NDR report and my DMV (CA) report and nothing shows up. I know I got maybe three tickets in the past but I really don't remember when and what the violation was. It was never reckless driving but I don't remember. Wrote an e-mail, hopefully someone will respond.
Actually, just got the e-mail response. They said that I did well by putting down what I remember to the best of my knowledge. Go ahead and do the same and you should be fine. Obviously, if you can't remember, it means that you are not getting one a year which is what they are checking for. Good luck..
