The quick answer to some of your safety-related questions: It is the "proper" use of checklists and "standard" operating procedures. With both crew checking, after doing the flow and backing up by the checklist, most of the mistakes are corrected. Good CRM, Airmanship, experience, and some freaking common sense make up the rest.

On getting info on this chat board: people dont come here to chat, they come to this board mostly to bitch. some come here to put their fellow pilots down to entertain their own Alpha type ego. But useful info is few and far between. I think the ALPA site has a section on becoming a pilot.
 
Once again thanks for all the additional information you guys/gals have posted. Yeah this is a paper for my Aviation Safety/Human Factors class here at SCSU. I’m getting further into the program so I am just trying to keep my grades up so HOPEFULLY I can nail some sort of internship, job etc...

As I am reading the information i am getting here, I am probably going to take my paper and lean it towards a more specific topic. Not just a general topic. If anyone else has any additional information, please feel free to share. I appreciate all who put the time and effort into helping me out. If I can figure out how to cite the information I receive here, I’ll be sure to give everyone credit.
