How does one obtain a NASA form these days?

FSDO office?

Snail mail from NASA?

Download from some web-site?

Of file the report on-line?

Had a brand new FO today that read back the wrong altitude and we ended up 1000' below assigned.

No traffic or TCAS warnings, but still, the Feds could get grumpy.

Yup, my fault as well, did not monitor him and the radio 100%.
 
Aye, that was quick.

The form is now printed and will be in the mail tomorrow.

Your not going to get violated unless the controller said "possible pilot deviation." Honestly, if we don't say it we can't do anything to you. Plus at our facilty FSDO has to be notified within 4 hours. If nothing happend as in loss of seperation, no controller is going to pursue that unless he lost seperation due to your errors.

Hmm, good news I guess.

In 27 years of flying with no violations, I am getting paranoid.

Therefore the NASA form.

God and Allah knows one have bent and broken every rule in the flying book, but not intentioanlly of course, unless life was at stake..

Climbing or descending? If climbing....not really a big deal...if descending...well, maybe. If you weren't notified of a problem(either on radio or told to call once you landed), don't worry about it, but definitely file a NASA report.

Doing the Fowee Arrival to MIA from the South.

And so we did..Except to 7000K instead of 8000 like the man said.

Ouch...NASA next.
