Medical mistake, How do I come clean?

I forgot to list on my medical a doctor's appointment (eye doctor) on my previous medical. When I went to get my medical the other day I flunked the eye and had to go get my contacts renewed and have my eye doctor fill out the form to turn in (he remembered to list the appointment) and marked that it was a corneal ulcer, (not disqualifying but should be reported)...I wouldn't worry but my previous AME got canned by the FAA and we have all been told our medical apps will receive EXTRA scrutiny in OKC. How can I come clean before they catch it on their own? I need like a NASA form for my medical....HELP!

I just went through something very similar to your situation. I would contact an aviation attorney. The FAA is very forgiving when you self-report. If you are a member of AOPA and suscribe to their legal plan, this would be a good place to start. If you would like more detailed information, PM me.

Regular appointments for physicals, dental exams, eye exams, etc. are NOT required to be reported. If the exam was for the corneal ulcer and its treatment and was not reported...then the above advice is sound.

The AOPA legal service plan is good . . . but of course check with your ALPA rep - they no doubt would maybe even have better attorney that they could help get you in contact with . . .

does it have to be a Dixie Cup or will any good quality brand name paper cup suffice?

I had something similar about 10 years ago - called ALPA aeromedical and Dr. Hudson handled it. Absolutely no problem.
