Hey guys,

Time is coming for me to get a new medical, and I'm getting all my paperwork together and such. Although I know the AME can/will answer this for me, I'm kind of curious what should be written down under the "Visits to Health Professionals" section, so that I can get that information together.

Specifically:

In the last 3 years I have a fair number of visits to dentists for dental work (couple fillings and such) and general doctors for colds / sickness (but nothing severe). Should these be written down?

What about visits to optometrists to obtain a new eyeglass perscription?

Also, I had my wisdom teeth removed. I know the actual surgery should be noted, but what about pre/post-surgery visits for consultation and examination?

Thanks much! I definately feel like I've made this more complicated than it needs to be.

My personal view of that question is that I only put down things that are more serious in nature. Routine dental work and normal colds and flu don't get put down. Surgery of any kind, major illnesses, and broken bones I would put down.
