do part 91 operators need to make reservations? If so I would assume it be published in the A/FD or a NOTAM issued.

Yes and don't forget to tip the maitre d

Just kidding.

I don't really understand your question.

A reservation in the airway system?
Or a reservation on the airport? (some are very congested)

You should only need to file a flight plan like everyone else. That's your reservation. The big computer looks at your ETD, requested altitude and route and then assigns something very inconvenient for you. Pretty simple.

We might get better service if we tipped.

certain airports require them, like Hyannis Port and/or Martha's Vineyard during the summer

there are others, too

check the NOTAMS, a lot of it is pretty short lived

Reservations are required all the time for 135 ops at LGA, JFK, and DCA (like it matters). Special Traffic Management Programs (STMP) go in effect all the time for airports where congestion is likely. For the summer its Nantucket and Martha's Vineyard. In the winter its all the Denver area airports. Also look out when NASACAR airforce comes to town. There will always be a NOTAM issued. For more info check these sites:
