So what can we do about it? Preventing poor judgment is an awfully daunting task. Stricter guidelines for training? Tighter rules? More enforcement? I don't know the answer.

I think changing the culture through good programs like the FAA Wings and AOPA safety awareness initiatives are the right apch. Gotta change the attitudes to have significant improvement. IMVHO, the system we have is really pretty good. We just need to work on the very unreliable, inconsistent portion - the people!
