Eye charts and the medical

I get a form from my FAA doc and have my eye doc fill it out. I think it's called a supplimental eye examination form or something like that.

I always have problems with those stupid machines and the open circle things.. This way I don't have to deal with them and actually use a real eye chart.


EDIT: The form is 8700-1 and is called Eye Evaluation
