Quality of the staff turning the wrenches plus the finanical support of the company to pay for the necessary parts I think is key.
 
There is a C550 operating out of Dallas TX, mostly
kids. So the regular door doesn't seem like such a problem.
There is a Citation Bravo out of Birmingham AL with
a wide door mod by Cessna. It was approx $250K.
The crew said it was the only modded Bravo around.

I fly this airplane, operated by Lifeguard Air Ambulance and owned by UAB Critical Care Transport. I would have to say that it is the Cadillac of the national Air Ambulance fixed wing fleet.

We all love the airplane and feel lucky to be flying it. The door is awesome, the interior design is perfect, the avionics are great, and the maintenance is impeccable. Probably the best of any airplane I've ever flown.

It's a 550B, a Citation Bravo. Door mod was around 250k. Average hours are around 25-30, but as those of you that fly air medical know that does not reflect the days, hours, and number of cycles/legs per day. We work our buts off. The plane itself flies around 1000-1200 hours a year and there are 6 full time pilots and a few part timers to fill in the gaps. We work a 4 and 2 schedule, with two shifts. I am on 4 day shifts, off 2 days, on 4 night shifts, off 2 days, and so on. It isn't a bad setup at all. I really like my job actually. Great airplane too.
