True, but that kind of freight comes by every once and a while.

It is a significant and lucrative chunk of the cargo market for the carriers and forwarders who have experience handling it. For carriers like FX as an example not their core which is why they basically junked the heavy weight cargo division and dumped the 74's when they bought FT. Doesn't make FX a bad carrier they just aren't a cargo airline.

96 inches... but the side door can do 108.

118" with no worries and up to 122" depending on the door and the ducting.
 
Lufthansa Cargo also chose to exit the nose-door business quite a few years ago, much to the pleasure of Cargolux. Basically their businessmodel said there'd be more profit to be had by switching from 747F NCD to MD-11F.
