Anyone ever miss an item on a checklist?

look up lights?

Yea, you're sitting there with the engine running, frolicking around with your charts, checklist, beer, whatever...meanwhile the plane creeps forward and your engine ingests the marshaller or some part off another airplane. You're bound to get a look up light on the CWS sooner or later.

try departing with the ignition only on one mag...worked fine to about 1500agl before the engine started running real rough and backfiring... scared the sh!t out of me and my instructor at the time.. suffice it to say, I've never again forgot to double check the ignition is on both after a run-up.

Forgot to turn the air on in the T1 and got the plastic jungle in back as we passed 10K. Hooked that ride.

I was doing a preflight, got in, cranked up, and went to run-up. I started to throttle up when I noticed my pepsi can still on top of the engine cowling. I must have taxied that thing like a champ because it didn't fall off. I have now added the pepsi check to my checklist.

In all the heavy aircraft, both civilian and military that I've flown, after the checklist are done, I recheck:

Trim, Flaps, Fuel Panel, Speed Brakes.

If that stuff gets done, there's not much else that can kill you.

I was on short final and my friend in the right seat forgot to buckle up suddenly we started hearing a banging sound it was the belt banging the door .

I got overloaded once (actually a BUNCH of times) when first flying the B200 and I missed "Yaw Damper ... OFF". I was dead on centerline, with no x-wind and everything loverly, so the boss let me land like that and try to turn off onto the taxiway. Laughing, he said he was comfortable letting me land with it on, and that'd be an excellent lesson and I'd never forget that one again ... and he was right. You couldn't have put a ten penny nail up my azz with an airhammer when I started stomping the left rudder and it wouldn't budge.

Did that in a CJ. Sad part was i didn't realize what it was and asked the other guy if he was on the rudder pedals again. Haven't done it since though.
