You beat me, Spidey! I was about to recommend a travel agent for a "Nassau form".

On the internet, I'd start with a visit to the AOPA website. They may have a "NASA form" there.

You RJDC Guys are correct, I can fly a 777. And since I am almost a Capt. on the RJ I should be a Capt. on it.
