I've actually flown into that airport, in the first picture, many times in a 727. Sucre, Bolivia (fine chocolate made there and most of the countries medical schools.) Problem though is the airport is at 8500 or 9300'. Also it's on the top of a mountain. The approach is only Day VFR, you descend into a valley, turn final, over two mountains, one which has been flattened on the top. RA goes down to 50' crossing the mountain, then pops back up to 300' or so and your over the threshold. Not any US carriers go in there, and is definately up there to one of the harder approaches.....
And the way the picture is facing...that's the only way for departure, Whereas the opposite direction looks a LOT worse than going this way.
