Template for Pt. 91 contract/agreement?

Hi-- I am new to corporate flying was wondering if any one could point me toward a template for an agreement. What should an agreement cover? Compensation, raises, vacation, benefits, retirement, call-out times, days off, cost on the road, uniform, etc.? If, you have any advise on must haves that would be great, too. I appreciate any help. Thanks a bunch….

Okay, I'm a little unsure what it is you are asking for. Are you asking about an "Employment Contract" or a "Flight Operations Manual."?
