Medical Insurance
All full-time SkyWest employees have the opportunity to select a health care plan from at least two options. All eligible employees may choose either a $500 comprehensive plan, or a national PPO plan regardless of where they live. Employees living in large metropolitan areas where an HMO is available, also have an opportunity to select an HMO plan.
Dental Insurance
Dental insurance is available for all full-time employees. The dental coverage includes an extensive network of providers. Participating employees are not required to coordinate benefits through a primary care provider before receiving services.
125 Flexible Spending Plan
The 125 Flexible Spending Plan provided by SkyWest offers employees two types of flexible spending accounts: a health care account and a dependent day care account. These accounts help employees to reduce their taxable income by setting a portion of their pay aside on a pre-tax basis for certain medical, dental, vision, or dependent care expenses.
Vision Plan
The vision plan offered by SkyWest provides employees with discount eye care and a nationwide network of optometrists, opthamologists, and opticians as well as the nation's leading optical retailer, LensCrafters. The plan also offers discounts on LASIK and PRK procedures.

RETIREMENT SERVICES
401(K) Retirement
Plan SkyWest offers a very competitive 401(k) plan to both full-time and part-time employees. The plan includes PRE TAX payroll deductions with an employer dollar for dollar match based on years of service. The plan, including any company contributions, is immediately 100% vested. Employees can choose to invest in more than 20 different funds and six lifestyle portfolios. All employees 18 years of age and older are eligible after 90 days of service.
Profit Sharing
At the end of each year, SkyWest makes an additional contribution to the 401(k) plan that consists of 5% of the previous year's profits. This contribution consists 100% of company dollars contributed to the plan on behalf of the employees. This amount is a contribution to ALL employees who have had two years of service with SkyWest and eligible employees do not have to be currently making contributions to the 401(k) plan to receive their portion of the profit sharing. The profit sharing is IN ADDITION to the matching program that SkyWest provides for employees enrolled in the 401(k) plan.
Stock Options
The SkyWest Board of Directors authorizes the granting of Stock Options to employees on a discretionary basis. Eligibility requirements are based on years of service at SkyWest. The number of shares granted to employees is determined by a formula weighted on earnings and longevity.
423 Employee Stock Purchase Plan
The 423 Employee Stock Purchase Plan provides SkyWest employees with the opportunity to buy SkyWest, Inc. common stock through payroll deductions. SkyWest pays the fees on the purchase, and employees receive a 15% discount when the stock is purchased every six months. Full and part-time employees are eligible to participate in the plan beginning with the next purchase period following 90 days of employment.
529 Educational Savings Plan
SkyWest offers a program to assist employees in saving for educational costs for themselves, their children, or grandchildren. This flexible 529-college savings program allows employees to save through payroll deductions on an after-tax basis for college expenses.
INCOME PROTECTION
Disability
The disability program helps to protect employees in unforeseeable situations where they may be unable to work due to illness or injury. SkyWest offers a short-term disability (STD) program that provides the employee with benefits one month after the employee has been out on a disability. Employees on STD receive 60% of their pay, up to $1,500 weekly with the benefit period lasting a maximum of 22 weeks. The employee's job and income determine the cost of this benefit. SkyWest provides a long-term disability program for an employee who is unable to work for more than six months. This benefit provides the employee with 60% of their pay, up to a maximum of $5,000 per month lasting up to age 65. Long-term disability premiums are paid entirely by SkyWest.
Basic Life Insurance
SkyWest provides all full-time eligible employees with a base amount of $20,000 in basic life insurance. In addition, coverage is provided for your spouse in the amount of $5,000 and a dependent child in the amount of $2,500. In the event of your accidental death, your beneficiary will receive an additional $20,000 benefit. This benefit is at NO COST to the employee.
Additional Life Insurance
Employees also have the option of purchasing additional life insurance for themselves and their family members. Employees may elect coverage in the amount of one to five times their earnings up to a maximum of $1,000,000. This supplemental group term life insurance also offers the following options: Portability or Conversion option, Living Benefits option, and Waiver of Premium.
Voluntary Accident Life/AD
Voluntary group accident insurance is available to SkyWest employees. This benefit protects employees and their family members against any covered accident in the course of business or pleasure. The AD&D Plan provides worldwide protection. You may elect employee only coverage, employee and dependent child(ren) coverage, and family coverage in increments of $25,000 up to a maximum of $500,000.
Legal and Financial Planning Services
Employees at SkyWest have the opportunity to participate in a comprehensive professional advisory plan that is like roadside assistance for your legal and financial needs. With this benefit, SkyWest employees receive legal services, identity theft services, financial planning and tax advice services. The benefit also provides employees with unlimited toll-free access to telephone attorneys in any state as well on-line legal and financial services.
MAKING LIFE A LITTLE EASIER
Direct Deposit
Direct deposit allows employees to have their pay check wire-transferred to their bank, credit union, or savings and loan at NO COST. This saves SkyWest employees the hassle of making a trip to the bank each pay day and is helpful to those who travel regularly as part of their job and may not have a chance to obtain and deposit their pay checks for several days.
Employee Assistance Program
SkyWest provides an employee assistance program at no cost to employees. The plan confidentially assists you and your family members in overcoming problems that may effect you at work or home, such as drug or alcohol dependency, marital difficulties, financial counseling or psychiatric care. Employees can also take advantage of life skills workshops such as money management and dealing with stress.

SkyWest provides full and part-time employees with convenient access to a web-based wellness program administered by My ePHIT. This benefit provides employees with customized health and life improvement programs, meal calculators, on-line life skills workshops and best of all, professional coaching from experts in fitness, nutrition, and self improvement. My ePHIT offers rewards for meeting personal goals as well as discounts on health and fitness products in their e-mall. They also provide resources to a wide array of health information, all for less than $4 per pay period for SkyWest employees.
