I've heard people ask for a radio check and get a response "I hear you 5 by 5" or something to that effect. What is it telling you versus loud and clear...
 
In the rest of the world they use 5-by-5. it's more of an ICAO thing, and it's supposed to be standard phraseology. the loud and clear is more something you only hear within the US.

It's a way of telling the receiving end of this radiocheck on a scale from 1 to 5 how good you hear. 5 being the best, 1 being the worst.

Normally i would just reply " I read you 5" but not the 5-by 5 though

On a scale of 1 to 5, they are ratings of volume and clarity.
 
Actually the 5 by 5 came from amateur radio.

In the amateur world when someone asks for a radio check they use the R - S - T scale. originally used for 'CW' (morse code)

Although in voice we only use the 'R' and the 'S'

R = readability
S = Signal
T= Tone
