I have searched this forum and have found most of my answers,however, I still have a few.

Regarding PIC, how does Netjets have an applicant disclose:
Dual Recieved
Dual Given
Solo

Regarding PIC, how does Netjets have an applicant disclose:
Dual Recieved
Dual Given

It specifically says on the application that dual given is to be included in your PIC, and Dual Received is to be excluded (even if legally logged as PIC in a plane in which you're rated).

No column for it. If it's legally loggable as PIC time per 61.51 (but not dual received), you can include it in that column.

The application has columns for PIC, SIC, CFI (dual given), Dual Received, Actual IFR, Simulated IFR, FE/Navigator time, Simulator time, Total time, and Time in last 6 months. Within each of those columns, you'll break it out between Single-engine piston, SE turboprop, SE jet, ME piston, ME turboprop, ME jet, and rotorcraft.
