An Air Canada 762-200 ran out of fuel at FL410. The fuel quantity indicator was MEL'd, and the aircraft was fueled improperly due to a faulty English-to-Metric conversion. He ended up using a forward slip on the 767 to make the landing work.

Minor damage to the aircraft due to a nosewheel collapse, but the aircraft is still flying with Air Canada today. According to the article, the mechanics sent to the gliderport to fix up the airplane ran out of gas in their van on their way there. Doh!

the only one I remember is the one CA1900 mentioned about the 767. It was confusion about converting gallons to liters or vice versa. Another good reason why the metric system should be adopted here in the U.S. It is simpler and easier to use!!!!

737 near New Orleans, landed on a levy.
 
Yup, that was TACA in New Orleans. (Although that one wasn't out of gas -- it ingested water and hail, and had a dual flameout because of it.)

Amazing landing; they flew the airplane back off the levee after fixing the engines!
