You can do this from the faa.gov site. Just click the licenses option on the left and it should guide you where you need to. I don't think there is a charge if you are changing the number from your ssn to another one.

I'd be curious to know when you get your new ticket/new number if it still has your SS# on it somewhere. My ticket ( old format, last issue '92 ) never had my SS# as a license #, but the SS# still got squeezed in near my name but I can't remember how this happened as I never would've agreed to it.

Anyone else have this happen ?
 
I requested a new certificate number a few months ago. My Social Security Number does not appear on my new certificates.

Also when applying of a new medical, I leave my social security number off the application.
