I recently lost my wallet which had my pilots license in it. I need to have one faxed to me ASAP. However I don't know the phone number to call. The FARs and FAA.gov only list an address. Was wondering if anyone knows the # of who I need to call.

Thanks

I don't have the book in front of me, otherwise I'd write the number here, but its in the front of the FAR's book under the Airmen Certification Branch address area. They can send you your airmen certs and medical info.

Replacement Certificate

Go to: http://registry.faa.gov/docs/8060-56.pdf

Replacement of Lost or Destroyed Certificates
For replacement of lost or destroyed certificates, submit a signed, written request stating your name, date and place of birth, social security number and/or certificate number, and the reason for replacement to the mailing address below. Click here for a form to request replacement of your lost or destroyed airmen certificate.

Mailing Address: FAA
Airmen Certification Branch, AFS-760
PO Box 25082
Oklahoma City, OK 73125-0082

Include a check or money order for $2 (U.S. Funds) for each certificate, made payable to the FAA. If your current address is listed as a Post Office Box (POB), General Delivery, Rural Route, or Star Route, please provide directions or a map for locating your residence. Allow 4 to 6 weeks for processing. Only one copy of each certificate can be issued. The original date of issue cannot be placed on a replacement certificate. Expired certificates cannot be issued; however, an expired CFI letter can be requested at no charge.

I know all the info that is listed in the FARs...I am looking for the number to call to get one faxed.

Jon,

Check your MOD (or whatever your dispatch/ help desk is) they are the ones that can fax you a copy. That is what we have at AA.
If not, goto your local (or a FSDO near you). They have the authority to issue what is called an "emergency field issuance".
Good Luck.

AAflyer,

Last time mine went through the wash.

Try This one
Its the Airmen Certification Branch in OK City

Thanks guys...You have all been very helpful. I think I am going to stick around and see what I can learn from this site. It seems very cool!

ooooohhh, , I hate that. That's why I started laminating mine. Costs a couple bucks at Kinko's and it's worth every penny
