I fly single engine jets, but for an application I have to list last instrument rating renewal date for both single and multi engine.

Last time I actually flew multi engine was when I took my ATPL in a PA-28-250. A couple of years later I got a B737 Type, but only flew the sim.

What do I put for renewal date for multi engine?

The instrument rating does not require a renewal. Recency of instrument experience may be established on the basis of several different methods, including an instrument proficiency check. You may also maintain proficiency by flying six approaches in six months, and performing and logging holds and tracking.

When you obtained your B737 type, you had to do so under the supervision and instruction of the instructor in the simulator. You should list this as your most recent multi instrument check, as it's an instrument checkride to ATP standards, assuming you have done no multi engine flying since that time.

A PA 28-250 is a Piper Dakota, a single engine piston powered light airplane.
 
NO, a Dakota is a PA-28-236, and a Charger is a PA-28-235.
The closest twin would be a PA-23-250 Aztec, as I think that he had a typo.
