*Clearance= Cleared direct A then direct B ..........and so on

*Fix A is 5nm north of airport. Winds favor rwy 18.

Does VMC/IMC matter?

Just takeoff and join the course between fix A and fix B
 
You takeoff, fly left traffic and head for fix A.

If you don't want to go to fix A and can maintain your own terrain separation, then you should advise ATC that you will be climbing on course direct fix B. You can do that during your readback to ATC or FSS. "We can maintain our own terrain and will be climbing on course today."
 
if its VFR throw everyone else a bone and pick up your clearence once you are in the air. Countless times I've heard people picking them up on the ground and stopping other IFR aircraft from landing or departing. If its VFR land and depart VFR if there is no tower. If it is IFR, taxi out, do your run up, then call the facility who gives you your clnc. They will give you a void time. If you are cleared to Airport XXX via fix route etc....fly the standard obsticle or dp for that airport, fly to the fix and fly the route.

To sum it up:

Uncontolled Airport:

1. If the controller does not provide you with a SID or heading to fly on for departure or does not tell you to proceed on course, you must comply with the DP on the 10-9......but.....

2. If it is VFR out, and you safely can fly to the first fix directly, you ask for a VMC climb from the controller and depart the airport maintaining VFR.

Controlled Airport:

Here's a fun one too: If you are at an airport with a tower and you were not provided a SID or heading or told to proceed on course by clearance, and the tower just tells you "cleared for takeoff", also without providing any guidance, the tower assumes you will fly the DP as written on the 10-9. Napa, CA (APC) does this and so does Sun Valley, ID (SUN). So be careful and know the DP just in case.
