Stay away from a Fed doctor and go to a specialist. Do not get a dignosis but accept treatment for whatever the specialist thinks you have. If the problem clears up than you won't have to check any of those little boxes for your medical. If you have an offical dignosis with a possible recurence then you could have a major problem with your aviation future.
