My first duty station in the army was in an attack helicopter company. About a month in country our XO and Security Control Officer either had their engine explode or they caught a missle up the ass. At any rate they were at 5,000' when it happened. It took the entire tail boom off the aircraft. They had plenty of time to think about what was inevitable. Other aircraft on freq. witnessed their thoughts as they plummitted. Our Maintenance Sargent and Crew Chief went in with them. It was not pleasant but it was an awakening for me. I always keep in the back of my mind "safety first". Keep your mind on what you are doing. Turn Volume UP.

Safety First  Pilot Error takeoff From Dallas

Safety First  Made approach to wrong runway hitting vehicle on go-around

Safety First  Icing at Washington National Airport

Some of you might have already heard these but they brought back memories for me. Safety First.
