Looking for a Discrepancy/MEL log form. Anybody have a form the could send or direct me to one?

If your 135, I think that you would have to create one and then have it approved by your POI.

With all those types in your profile, you should have seen enough of discrepancy logs that you could create one from scratch in your sleep while dreaming about taking your 7-5 type ride, balancing your checkbook and solving world hunger.

Or ditto that above and walk to your airplane of flight bag, pull out your GOM and photocopy the discrepancy log approved for your current certificate.
