Does anybody know of a good place to find a list or reports of CRM related accidents/incidents? I looked on the NTSB site, but its hard to find anything there... Thanks

use yahoo and go to jack hunt ntsb reports. click on to aircraft accident reports. you may have to read through the reports especially the last few pages.

TIP: most (cfit) controlled flight into terrain accidents/incidents are caused by pilot/crm issues and error.

Most accidents/incidents are still cause by pilot error. controllers get to go home every night.
 
Thanks Rez, that is good stuff. Anyone know how to get CRM training aids, the stuff we have is old. Looking for accidents like NWA 255 Aug 87, UAL Portland, OR 1978, etc
