1.0 INTRODUCTION:

The revision to this Advisory Wire (initially issued March 10) is to provide Operators with the status of the investigation into this accident and to provide details of a finding related to the hand held microphone audio jack assembly that was installed on the pilot and co-pilot control columns of this aircraft.
2.0 INFORMATION:

A Challenger 600 aircraft overran the runway during take off from the airport in Tupelo, Mississippi on 09 March 2005. The weather conditions at the time were plus 6
°C (43°F) with light rain and good visibility. The aircraft was departing with five passengers and two crew on board. No injuries were reported.
During the event, the take-off was rejected, and the aircraft overran the end of the runway by approximately 140 feet. The nose landing gear collapsed, and the area of the forward fuselage sustained damage. The aircraft Captain reported that when he attempted to rotate the aircraft, the control column would not move aft beyond the neutral position.
The accident investigation team has discovered that it is probable that the control column movement was restricted in the aft direction due to a microphone jack receptacle on the co-pilots control column that was found bent downwards.
An inspection of the co-pilot control column revealed that the hand held microphone jack receptacle (installed with two screws through a bracket on the lower side of the receptacle) mounted on the lower aft side of each control column approximately 1” from the bottom of the column, was bent downwards. (See photos below).

With the receptacle in this position, when the control column was pulled aft, the receptacle contacted the control column cover and prevented further aft movement of the column. It is not known yet what bent the microphone jack receptacle. The origin of the installation is not known at this time.
3.0 ACTION:

Bombardier recommends that if microphone jack receptacles similar to the one described in this AW are installed on their aircraft, Pilots should verify the security of the installation of the receptacle as part of the pre-flight visual inspection of the cockpit. Bombardier continues to support the NTSB with this investigation and we will keep you informed of any further developments.
