...aaaaaaand of course, there wouldn't always be an NTSB report, as in a student or private without instrument getting into unforcasted IMC and using his/her emergency authority to execute an instrument approach would be legally IMC and could log the actual time as PIC. Legally.

91.3 works well until you die...thus the NTSB report.
