FWIW, I take it to mean that you need radar and either DME or an ADF.

At first, I thought it was that way, but you can fly the approach without RADAR if you've got DME because you can still ID the fixes (including the holding fix off of the M.A.) So I took it to be:
RADAR and ADF or DME Required - means RADAR and ADF can be Replaced by DME
someone should really talk to the FAA about their grammar
