Hello everyone. I am finally back from Afghanistan and it is nice to be home. My concern is, I will start my Fixedwing PPL training and I need the medical certificate, I was told the Army Flight Surgeon at my unit, at least one of them was authorized to sign off on it. I completed my yearly flight physical last week and everything is still within standards for a Class 2. And tomorrow I will go see the Flight Surgeon, but, don't know if I need to take the FAA form 8500-8 or does he have access to it somehow? Is there a webpage I can download this form? I tried a few searches but seems like it is not available for downloading due to fraud issues.FAA FORM 8500-8 FAA FORM 8500-8
 
Call the flight surgeons office and see if he has the forms. My experience is that he allready does.

It's not available for download. Its one of the few forms left that are done by hand in multiple copies. Another is the aircraft registration application.
 
Thanks everybody, I was able to get my certificate today at the base and yes the Flight surgeons keep them because they are control forms. Thanks again for all the quick responses.
