This is a fascinating article I found via /. that is in effect an accident report of the Christmas 2004 meltdown of Comair's SBS crew scheduling system.
It's interesting to note that 8 days before the crash Comair announced a 3Q2004 profit of $25.7 million. The computer crash cost around $20 million.
Like a lot of accident reports, in 20/20 hindsight the incident seems obviously preordained:
