We went through something similar a couple of years ago in frac/135. When you have subcontractors who subcontract, who has operational control? Remember the Expressjet that spent the night on the ramp in MN? Mesaba did their ground handling and didn't have an agent available. They kept passing the blame back and forth. I think both sides thought they had acted correctly, but nobody knew/knows where the buck stops. Allowing only one operating certificate per holding company would go a long way to solving the problem. But airlines would scream bloody murder if you try. And they have the money (read politicians) in their back pocket to prevent real reform.
