More NetJets App. Questions

Received the app in the mail yesterday. After filling it out all day today they asked us to fill in Part 1 of the FAA PRIA worksheet even though in the instructions it says it should be filled out by the Air Carrier (Form 8060-11). The other instruction sheets say for the FAA to fill in the blank. Is this what everyone did? I'm probably being a little picky but don't want to get the app back in the mail. Also on the crewmember data form at the bottom it says (pilots only) and the ATP__________ Date of last medical and cert. #. What do they want by ATP? yes or no or date issued? lastly, on the app for "reason for leaving" do they just want a box checked unless you were discharged?

On the FAA PRIA worksheet, I did fill in the info. On the crewmember data form I wrote in the date. I wrote in a reason for leaving for every job. My app was approved. Hope that helps.
