Just wanted to know if any corp operators hand out a special request questionaire to their passengers. Ex: special needs and catering request or anything they just might want.

If you do, please give some ideas how it is formated. And let me know if it is effective.

Well, we have "Passenger Profiles" in FlightPak... unfortunately most aren't filled out, but some are... Usually our dispatchers contact the office of the executive in question and try to get info on what they like as far as catering or any other special requests... Then they enter it into the profile and we try and use that as a guideline for future trips if no special catering was requested...
 
We make sure our frequent passengers fill out a simple profile form that includes their preferred meals, drinks, etc.. for each meal of the day... All our pilots keep the pax profiles with them and they are kept in the aircraft logs also.
 
Having a guest comment card, to be filled out during or after the flight, might also be an idea. It accomplishes 2 things, 1, it lets you know how the flight went, and 2 you might leave a comment section or something that asks what their favorite meals and such are.

Another thing to have handy is a SEAT BELT EXTENDER!! Both times I have needed one; I had no idea where it was. It's awfully embarrassing for me and even more so for the fata$$ that needed it!
 
All of our "special" information is handled when the customer or broker makes the arrangements for the trip. Our regular customers' particular needs are normally recognized by us, such as the lawyer that likes Starbucks coffee, or the labor head who wants lots of diet cokes.

If we are doing a round trip or a layover with the customer, we inquire what the customer would like for the return trip as they get off the plane.
