knowledge test pass dates?

I have an interview coming up in a few weeks, but I was sent a supplement to the application that asks when I passed my commercial and ATP writtens. Since I already have both the commercial and ATP certificates, I didn't keep track of when I passed the knowledge tests. Didn't think I needed them any more. It's been quite a while ago. Does anyone know of any one I can contact or any web sites that will provide this information?
 
I would imagine they only ask those dates if you have not received the rating, i.e. if you took the ATP written but not the ride, enter the date.

Although thats only a guess, I can't imagine any reason they'd care otherwise...

Might want to call the company and ask.
