Just a general culture question here....in the USA some private airports or private runways have their own instrument procedures, that the owner uses or you can ask for his permission to use too, but how do they get those procedures?

Do they have to pay the FAA to make one for them?

I worked at a place that had a few at different small airports that we owned. We had to work with Jepp (pay them) to design the procedure, then get the FAA to approve it.
 
FAA Process for IAP design

Check out link to FAA web site. You can pay the FAA to design an approach for you, or a private company (like Jeppesen) can design as well. FAA probably costs more. In either case, FAA must flight inspect (Flight Check) the procedure and requestor must pay for the initial flight inspection and any periodic checks thereafter.
