1. It states that you can't use minimum lower than derived from the table. If the published minimum is higher than calculated, does the published minimum apply or does the calculated minimum?

2. In Canada, if the standard minimum is applicable, than a 'sliding scale' can be used. i.e. if 600' and 2SM applies, than 700' and 1 1/2SM or 800' and 1SM can be used. Is there anything similar in the FAR's and if so, would it be applicable to the above OPS SPEC?

3. Do the weather requirements have to be forecasted in a TAF or can an Area Forecast be used?

Calculated. Read your question again. It says you can't use a lower minimum, it doesn't say you must use a higher minimum. Standard is 200 and a half or 400 and one added to the straight in minimums.

Not sure what you are talking about there.

You need a TAF. An airport without a TAF will most likely say n/a in the "for filing as alternate" section. If you are using the table you mentioned above, you ignore the minimums listed here, but still pay attention to the n/a as it can be a good heads up.
