Quantum in Phoenix is one of a few that are recommended...you might also try Hillsboro Helicopters or Helicopter Adventures Inc.

I took a few lessons from a guy who got his stuff done at a school called Vortex. It was on the Gulf Coast in either Missippi or Louisianna. He seemed pretty happy with the school.
