chq is 60/mo for single health and dental, vision is free. 16 a mo to add dependent onto dental and to add to health depends on how many dependends, not sure since my wife has insurance through her work

This isn't the whole picture... it depends on where you live, i.e., Utah, Colorado, California, or the Northwest. It also depends on PPO, HMO or our 'wonderful' new Health Savings Account (HSA) The United Healthcare PPO is 99/mo single, 103/mo 2-party, the family is 252/mo.

HSA is 44/mo single + whatever you contribute, 86 for 2-party + contributions, family is 105 + contributions/mo.

The HMO's for single people vary between state from 70/mo-103/mo. 2-part b/w 160-212/mo, family b/w 103-190/mo.

CommutAir is $26/week for family PPO medical through BlueShield, including a vision plan that pretty much nobody accepts. It was free up until a couple of years ago, when it went up to $16 a week, and it recently jumped again to $26 a week.

Dental is around $5 a week for one; not sure on family.
 
No premiums at Horizon, unless you have dependents...
 
Thanks for the help guys. Damouse your right, I guess I just assumed that most of the plans would be PPO. Know what happens when you ass-u-me.
 
Trans States is $107 a month for single, i don't even want to know what a family would be. Dental is around $15/mo and it is completely useless.
