I'm an active USMC pilot looking to apply to a hometown AF Reserve squadron. They requested for me to complete the following forms:

-Cover letter
-Resume
-Ltr of Rec
-Flying history report
-AF Form 942 + 1288
-Form 8
-OPR's
-AF Med Form 1022
-DD Form 214

I wanted to ask if anyone could explain the proper format for the Cover letter/Resume/Ltr of Rec/Flying history? I'm also not familiar with the remaining AF forms or where to find them. Could anyone give their advice on the best way to complete the application process, advice for the interview, any other good info to know? Thanks a bunch.

I'll try to translate some of these items:
For the cover letter and resume, the AF has a writing manual called the "Tongue and Quill" with examples in it. If you search the AF pubs military webpage, you might be able to find it. If not, send me a PM and I can scan in copies of those pages.
Flying History Report - this is a computer generated form which lists all of your flying time in various categories (Primary/Sec/IP/Evaluator, etc). With your USMC logbooks, you'll have to do the math yourself to create something similar.
AF Form 942 - this lists all of your checkrides and grades (not sure if there is a NATOPS equivalent)
Form 8 - Checkride gradesheet/comments/downgrades (not sure what the NATOPS equivalent is)
OPR = FITREP
AF Med Form 1022 - is this supposed to be 1042? This is your med clearance to fly after your physical
DD Form 214 - This you get after you separate from active duty.

Bottom line - do the best you can with getting "equivalent" forms. If you have questions, contact the unit you are applying to and let them look over your stuff. Most will be happy to provide feedback to make sure you are providing the info they want.
