N reg aircraft Pt 91, one engine decides to take a rest, and thus force descent.

Who do you send the occurance report to, (in Ganders patch).

Is there a standard report form.

I don't have a copy of the North Atlantic chart but there is listed on it's communication page a website for Contingencies and such. I take it that you were out of radar coverage and had to do some kind of contingency. I would start there. Might want to speak with the FSDO that approved your operation and see what they want and if they have any information for you.
